Bring These to Your Appointment
- Valid, unexpired photo ID for each signer, such as a California driver’s license, California ID, U.S. passport, or other legally acceptable identification.
- Printed documents, unsigned. The notary must witness required signatures.
- Witnesses, if required. Witnesses should be adults with valid ID and should not be named in the document.
- All signers present and able to sign willingly. For hospitals and care facilities, please confirm check-in rules and signer capacity before the appointment.
Before You Call
- Know the document type, such as loan documents, power of attorney, healthcare directive, affidavit, trust, deed, or real estate paperwork.
- Confirm who must sign and whether witnesses are needed.
